Referral Information
Referrals are obtained by faxing to our office at 705-743-1449, through patient’s family doctors, nurse practitioners, walk-in clinics and hospital visits. We ask that each referral have up to date and correct patient information, clearly stated on the cover page of the referral ie. phone numbers, address, OHIP number and date of birth. These referrals are then prioritized and booked accordingly by either; an appointment card sent out in the mail or a direct phone call to the patient or next of kin.
