The Medical Centre Primary Care-Embedded Memory Service
The Medical Centre Primary Care-Embedded Memory Service is a multi-disciplinary, inter-agency Memory Assessment Program designed to assist Family Physicians and Nurse Practitioners in the timely assessment and diagnosis of memory concerns. The program has been developed and runs in collaboration with the Alzheimer Society of Peterborough.
A referral must be made by a Physician or Nurse Practitioner in order to get an appointment.
